We did something like that for our LV-11; I vaguely remember italics being 
implemented by "sloping" the text, and bold by offsetting by a dot.

Mind you, that Versatec spent most of its time plotting biorhythm charts 
for the operators and their GFs; well, this was the 70s, man...
