I'm familiar with this sense of "monitor" in that Apple used it for the 
system firmware on the II series, but I always thought it weird when the 
same term was used, say, for a lightweight operating system loaded from 
disk (I've seen CP/M expanded both as "control program for microcomputers" 
and "control program/monitor" and I think they may have both been official 
at different times).
