Found it, my copy appears to be the first issue (V6 era rather than V7), predating the scan above.  Two standard punch holes are visible in the top edge of the printout, presumably the original was bound with a couple of brads in a flipbook.

I'll plan on scanning this in the coming weeks.  Unfortunately the cover and title page aren't in this copy, but I think everything else is there, the page numbers are sequential except one that coincides with what was probably a blank page in the original.
