[TUHS] Updated installation instructions for v4 and v5, including B

Hi everyone,
I have expanded my installation guide for UNIX v4 and wrote a similar one
for v5 as well. While I was at it I also got my reconstructed B compiler
running on both systems, there are instructions for that as well
